Care Manager – Acute Care Facility Assignment Duration This is a 13-week contract position. Schedule Day Shift: 5×8-Hour (08:00 – 16:30) Job Description The Care Manager will be responsible for performing care coordination and discharge planning in an acute care setting. The role involves assessing patients’ needs, coordinating care, and ensuring timely discharge processes. Responsibilities Conduct care coordination and discharge planning for a case load of up to 25 patients.
Utilize Epic charting system for computerized documentation. Assess patient needs and develop comprehensive plans of care. Ensure compliance with CMS guidelines and HIPAA regulations. Collaborate with interdisciplinary teams to provide high-quality patient care. Required Experience / Certifications / Licensure Minimum 2 years of acute medical care management experience. CA State License is required; pending licenses accepted, provided licensure is obtained by the start date.
