As a Hospice Social Worker, you will collaborate closely with nurses, physicians, chaplains, aides, and volunteers to ensure patients and families receive holistic, person-centered care that honors their wishes, dignity, and comfort. Essential Responsibilities Conduct comprehensive psychosocial assessments of patients and families.
Develop individualized care plans based on identified psychosocial needs. Provide emotional support, counseling, and crisis intervention to patients, families, and caregivers. Facilitate discussions regarding coping strategies, grief, loss, end-of-life concerns, and advance care planning. Connect patients and families with community resources, financial assistance programs, support groups, and other services as needed.
Collaborate with physicians, nurses, chaplains, hospice aides, and other interdisciplinary team members to ensure holistic patient care. Participate in care conferences and contribute to care planning and goal setting. Assist families with discharge planning, placement options, and transition of care when appropriate. Maintain accurate, timely, and compliant documentation in the electronic medical record.
Provide bereavement support and follow-up as assigned.
