Join an established team of 3 Internal Medicine physicians, 1 Internal Medicine APP, 2 Family Medicine physicians, and a PRN Internal Medicine PA Staffing includes 1:1 MA support, a daytime regional RN triage team, and an evening RN triage team managing after-hours calls. Onsite services include lab and x-ray. Specialists rotating into the clinic include Podiatry, Neurology, Sleep Medicine, and Orthopedics, with a behavioral health clinic located upstairs.
Deliver comprehensive, patient-centered primary care, including high-quality diagnostic and treatment services, to improve the health and well-being of the community. Coordinate seamless care transitions for patients, managing specialized consultations, inpatient admissions, and collaborating with case management for successful discharge planning. Lead or actively participate in continuous quality improvement initiatives to enhance the efficiency and cost-effectiveness of care delivery within the clinic and the wider system.
Maintain complete and timely patient documentation and electronic medical records to ensure compliance, accurate records, and support the financial integrity of the practice. Ability to practice autonomously and demonstrate comfort managing a solo outpatient primary care panel within a clinic environment.
