About the Position: This position is located at the 55th Medical Group, Offutt Air Force Base in Nebraska. This is a Direct Hire Solicitation A recruitment or relocation incentive may be authorized. Salary negotiation may be available for those candidates who are new to federal service. Student Loan repayment may be authorized. Non-Federal work experience may be creditable for leave purposes. Major duties: Coordinate healthcare services, referrals, and resources across military and civilian providers to ensure timely patient care.,Analyze complex medical, psychosocial, and administrative problems to recommend effective solutions and improve outcomes.,Provide comprehensive assessments of patient health needs and develop individualized, patient-centered care plans.,Assess patient progress, collaborating with multidisciplinary team and patient/caregiver to address concerns and guide decision-making.,Educate patients, families, and the healthcare team on case management, treatment options, insurance benefits, and available community resources.
