Location: Avera Diabetes Services-Sioux Falls Worker Type: Regular Work Shift: Primarily days with night, weekend and holiday on-call involvement (United States of America) Pay Range: The pay range for this position is listed below. Work where you matter. A Brief Overview Provides individual and group diabetes self-management education, conducts community and health professional education related to diabetes, and serves as a resource for the nursing staff relative to diabetes education and basic nursing care of the diabetes patient. What you will do Plans, conducts and evaluates system diabetes disease management processes and systems.
Serves as a clinical resource person for providers and nursing staff related to the care and education of diabetes patients. Supports clinics in disease management and access to diabetes education. Identifies opportunities for program development and participates in special projects as assigned. Develops, conducts, and evaluates training program for clinic staff to provide diabetic education. Collaborates with diabetes management and education resources across the system.
Maintains expert knowledge in diabetes education and case management tactics. Essential Qualifications The individual must be able to work the hours specified. To perform this job successfully, an individual must be able to perform each essential job function satisfactorily including having visual acuity adequate to perform position duties and the ability to communicate effectively with others, hear, understand and distinguish speech and other sounds.

Required Education, License/Certification, or Work Experience: Registered Nurse (RN) - Board of Nursing An active license in the state of practice Upon Hire Preferred Education, License/Certification, or Work Experience: 4-6 years of experience in disease management or case management Experience in disease management specific to diabetes mellitus Expectations and Standards Commitment to the daily application of Avera’s mission, vision, core values, and social principles to serve patients, their families, and our community.
