As a Community Support Specialist , you will work directly with individuals experiencing mental health and/or substance use challenges , many of whom are homeless or at risk of homelessness . In this role, you’ll help connect clients to housing, healthcare, and critical community resources —empowering them to move toward independence and long-term success. This is a highly hands-on, community-based role —perfect for someone who enjoys building relationships, advocating for others, and making a real difference every day.

Housing & Stability Support Help clients find, secure, and maintain housing Build and maintain relationships with landlords and housing partners Assist with housing applications, resources, and financial assistance options Case Management & Advocacy Provide community-based case management to individuals in supportive housing Connect clients to healthcare, employment, education, and social resources Advocate on behalf of clients to ensure access to needed services Skill Building & Recovery Support Help clients build independent living skills (budgeting, hygiene, daily routines) Support development of coping skills and recovery strategies Provide education on mental health, substance use, and wellness Collaboration & Care Coordination Work closely with treatment teams, healthcare providers, and community partners Participate in treatment planning and team meetings Coordinate services to support long-term success and stability Documentation & Follow-Up Maintain accurate and timely documentation of services Follow up with clients and referral sources to support engagement Assist with discharge and long-term recovery planning Crisis Support Support clients during crisis situations Help connect individuals to appropriate crisis resources and services What We’re Looking For Education & Experience We welcome a variety of backgrounds!
