Provide nursing assessments, care coordination, health education, and clinical support that promote health, recovery, and overall wellness. Support medication management, medication education, and treatment adherence while helping individuals achieve their goals. Collaborate with interdisciplinary team members, healthcare providers, families, and community resources to ensure coordinated care. Monitor health concerns, treatment progress, and emerging needs while supporting timely intervention and follow-up care.

Educate individuals on medications, health conditions, wellness practices, prevention strategies, and self-management skills. Coordinate referrals, communication, and continuity of care among internal and external providers. Advocate for individuals by helping reduce barriers to treatment, healthcare access, and support services.
