Facility: Dayton Children's - Tech Town Department: PFK Care Coordination Schedule: Full time Hours: 40 Job Details: The Outreach Coordinator PFK has the following essential job functions: • Screens for healthcare needs and connects to related programs and resources. Interacts daily primarily via telephone and electronic communication. • Initiates telephone contact and completes a health risk assessment.

Escalates complex or urgent needs to a supervisor. • Connects patients to the right program once needs are identified. Develops individualized self-management skills to patients and families through education and resource provision. • Provides organization of electronic communication related to survey results of the health risk assessment. Documents within the electronic medical record. • Participates in orientation and continuing education of staff and students as appropriate.

Department Specific Job Details: Education High school diploma required Experience Background in health care, public health, or related clinical field preferred Skills/Competencies Excellent customer service skills with the ability to communicate effectively through multiple mechanisms including phone and writing required Schedule: full time, 40 hours day shift, 8:00 AM - 4:30 PM no weekends or holidays serving the Health Risk Assessment screening program with Partners for Kids
