$5,000 Sign-on Bonus The Assertive Community Treatment (ACT)/lntegrated Dual Disorder (IDDT) Team is a multi-disciplinary, community based, treatment team providing a broad range of supports to individuals with severe and persistent mental illness. The duties for this position include providing co-occurring, crisis, and case management support to adult consumers, their families and natural support groups.
This individual will perform assessments, develop treatment plans, manage referrals/linkage, monitor progress, integrate treatment, coordinate benefits and maintain the consumer record. MINIMUM QUALIFICATIONS: – Bachelor’s degree or Master’s degree in social work, psychology, counseling from accredited program – Eligible for licensure with the State of Michigan – Certified as an Addictions Counselor or eligible to acquire certification within 3 years DESIREABLE QUALIFICATION: One year experience with mental health services To apply, please click the link: https://ccmh.hirescore.com/ EOE
