Physical Therapist (PT) - Home Health Job Description We are seeking a dedicated Physical Therapist to join our Home Health team. The ideal candidate will provide high-quality physical therapy services in patients' homes, focusing on rehabilitation and enhancing mobility. Responsibilities Conduct comprehensive evaluations of patients' physical abilities and develop personalized treatment plans. Implement therapeutic interventions to improve patients' mobility, strength, and overall function.

Monitor and document patients’ progress, making adjustments to treatment plans as needed. Educate patients and their families on home exercise programs and mobility techniques. Collaborate with interdisciplinary teams to ensure optimal patient care. Qualifications Current Physical Therapist license in the state of practice. Experience in home health therapy preferred. Knowledge of OASIS documentation requirements.

Strong communication and interpersonal skills. Ability to work independently and manage time effectively. Schedule This position offers a full-time schedule consisting of five 8-hour shifts from 08:00 AM to 04:30 PM. Additional Information Mileage will be reimbursed at the IRS rate.
