If you are an experienced Health Visitor, we would love to hear from you. We are looking to recruit motivated practitioners, to join our staff bank, with initiative and drive, and who are keen to contribute to the further development of our services. The post holder will lead and ensure delivery of Healthy Child Programme with a focus to implement early help/intervention so reducing health inequalities and improving health outcomes for individuals, families and the community.
Health Visitors in NELFT will apply a family focused public health approach to Universal programmes and targeted packages of care and interventions as stipulated by the Healthy Child Programme. They will deliver intensive programmes for the most complex and vulnerable children and families., The key responsibilities of this post, include but are not limited to: Use child and family centred approaches to address health inequalities, including the promotion of good health, the prevention of ill health and the implementation and evaluation of individualised programmes of care.
This should involve the service user by developing child/family care plans in partnership with families. Monitor the health of children under five years of age and their families, and to involve the family in promoting optimum health and development of all children. Use strength based, problem solving/solution focused strategies and motivational interviewing skills to enable families to develop behaviour change strategies.
Undertake home visits and regular reviews to support vulnerable families, groups and individuals giving information and support. Be aware of deviations from the normal in health and behaviour, and to intervene to protect vulnerable children and adults by prompt action, referral and working with other agencies as part of a specialist team.
