House Call Nurse Practitioner Brooklyn, NY Full-time Position Overview A large multispecialty group in NYC is seeking a Nurse Practitioner (NP) to provide high-quality, patient-centered care in patients’ homes and nursing facilities. This role is ideal for providers who enjoy holistic care, chronic disease management, and direct patient engagement . Key Responsibilities Patient Care & Evaluation Conduct comprehensive assessments, including physical exams, medication reconciliation, and medical history review during home or nursing facility visits Diagnose medical conditions and develop individualized treatment plans Prescribe medications and manage chronic illnesses Care Coordination & Collaboration Collaborate with physicians, specialists, and multidisciplinary teams to ensure continuity of care Provide referrals and coordinate follow-up with other healthcare providers Patient & Family Education Educate patients and families on disease management, lifestyle modifications, medication adherence, and preventive care Documentation & Follow-Up Maintain accurate and timely documentation in the EMR/EHR Schedule and conduct follow-up visits to monitor patient progress and adjust treatment plans Emergency Response & Community Engagement Assess and manage urgent medical issues during visits Participate in community outreach and patient education initiatives Qualifications Master’s Degree from an accredited NP program (Family or Adult/Geriatrics focus) Current New York State NP license National Board Certification (AANP, ANCC, or ONCC) Due to patient population, client is seeking providers who are bilingual in Russian or Chinese Experience with EMR/EHR systems preferred Strong clinical judgment, communication skills, and ability to work independently Compensation & Benefits Competitive salary: $130,000 – $200,000 annually Comprehensive benefits package EMR training and ongoing professional development Supportive multispecialty group with robust provider infrastructure Organization & Lifestyle Large multispecialty group with offices throughout the five NYC boroughs Over 140 physicians and 100 APPs Abundant public transportation options Access to NYC’s cultural, culinary, and entertainment offerings
