Care Homes Clinical Lead (Advanced Practitioner)

We are looking for an experienced and motivated manager to clinically lead the PCN multidisciplinary Care Homes Team across Rushcliffe. This is an exciting opportunity to combine clinical leadership, operational management, and hands-on patient care, working closely with GP practices, care homes, commissioners, and system partners to deliver high-quality, proactive care for care home residents.

About the role You will: Lead and manage a multidisciplinary team of clinicians, GP's, ACP's from nurse and paramedic background, nurses, and care coordinators. Deliver the Enhanced Health in Care Homes (EHCH) model across the PCN. Provide clinical support to the team as well as operational management. Support service improvement, quality assurance, and clinical governance. Use data and insight to drive performance and improve outcomes.

Working closely with Rushcliffe GP practices and care home staff, the postholder will deliver proactive care and support to older people living in residential care. This role bridges health and social care boundaries to deliver the NHS Enhanced Health in Care Homes (EHCH) framework., Operational Management & Leadership Service Delivery: Manage the team's delivery of the NHS Network Contract DES Enhanced Health in Care Homes specification.

Staff Management: Conduct staff 1-2-1s, annual appraisals, clinical notes audits, and medication audits. Accountability: Support the team, hold staff accountable for action delivery, and directly address poor practice when necessary. Financial Efficiency: Ensure the care home team operates efficiently and provides value for money as a centralized PCN service on behalf of the 11 GP practices. Strategic Input: Contribute as an active member of the PartnersHealth senior leadership team, balancing operational duties with strategic organisational positioning.

Clinical Excellence & Practice Clinical Support: Provide hands-on clinical care and autonomous decision-making for care home residents following comprehensive assessments. MDT Collaboration: Contribute to weekly care home rounds, support the team and contribute to clinical decisions when needed. Safe Prescribing: As an independent prescriber prescribe safely and appropriately in accordance with local non-medical prescribing policies and protocols.

Governance & Risk: Apply strong clinical governance and risk management principles to all areas of practice, highlighting risks and benefits when developing new ways of working. Data, Reporting & Service Development Data Utilization: Interpret data and local intelligence to drive service improvements, support primary care resilience, and reduce avoidable urgent and emergency service activity.

Performance Reporting: Provide data-driven reports to commissioners, the PCN and to the PartnersHealth Board to evidence compliance and delivery with local and national specifications. Innovation & Tech: Promote the adoption of digital technology where possible to enhance patient safety, increase operational efficiency, and establish best practices. Evidence-Based Care: Assist in developing an evidence base for health needs-led service planning, commissioning, and identified research developments.

Communication & Professional Standards Advocacy: Champion the voices of residents, care home staff, and the care home team across integrated health and social care systems. Visib
