Green 5 West Heart Failure/Cardiac Telemetry Unit is a 29 bed Joint Commission certified Heart Failure specialized care unit. The primary focus of the unit is on the care of Heart Failure cardiac patients who require continuous cardiac monitoring pre and post electrophysiology procedures and patients with newly diagnosed or chronic congestive heart failure.

Common admitting diagnoses include pre and post cardiac catheterizations that require no interventions, congestive heart failure patients, pre and post pacemaker/AICD implantation with suspected or significant potential rhythm disturbances and acute coronary syndrome. Nursing activities include monitoring, rhythm interpretation, fluid and electrolyte monitoring, inotropic therapy, respiratory status, pre/post procedure care, medical management, rehabilitation initiation, and comfort/palliative care measures as indicated.

Patient and family education and emotional support related to the patient’s condition and treatment is an essential element in the care the team provides. The RN Clinical Charge Nurse is responsible for directing and overseeing the daily activities of the unit on an assigned shift to assure successful safe delivery of care. The Charge Nurse is a caregiver and often takes modified patient care assignment.

They practice in a clinical environment that is administered by Nurse Managers and other leaders and is supported through the WellStar Shared Governance Model. The framework for practice is steered by the WellStar Professional Nursing Practice Model and evidence based practice and research. RN competencies are derived from these models and supported by the WellStar Core Competencies. The RN Clinical Charge Nurse is a proactive member of an interdisciplinary team of licensed and unlicensed care givers who ensure that patients, families and significant others receive individualized high quality, sa
