Are you an experienced Registered Nurse with a passion for leadership, rehabilitation and delivering outstanding patient care?HCRG Care Group is proud to have been chosen by Medway Council to deliver the Intermediate Care and Reablement Service across the Medway community from 1 October. As we launch this exciting new service, we are looking for a Clinical Lead Nurse to play a pivotal role in supporting both our Home First and Bedded Intermediate Care pathways.This is a unique opportunity to join a service at the very start of its journey and help shape how care is delivered across Medway.

Working alongside senior therapy colleagues and multidisciplinary partners, you will provide clinical leadership, support service development and help ensure local people receive high-quality, person-centred care that promotes independence and positive outcomes., As Clinical Lead Nurse, you will provide visible clinical leadership across the Intermediate Care and Reablement Service, supporting the delivery of safe, effective and responsive care across both community and bedded rehabilitation pathways.

You will act as an expert clinical resource for nursing teams and wider multidisciplinary colleagues, ensuring care remains person-centred, evidence-based and focused on helping individuals achieve greater independence.Working closely with operational and clinical leaders, you will support the day-to-day management of patient flow, capacity planning and service delivery. You will play an important role in facilitating safe discharge planning, preventing unnecessary hospital admissions and ensuring people receive the right care, in the right place, at the right time.You will also contribute to workforce development, clinical governance and quality improvement activity across the service.

This includes supporting supervision, developing colleagues, maintaining high standards of professional practice and helping drive a culture of continuous learning and improvement. Registered Nurse (Adult) with current NMC registration Degree in Nursing or equivalent qualification Evidence of continuing professional development Leadership or management training, or willingness to undertake Significant post-registration experience within community, rehabilitation, intermediate care or integrated care services Experience managing patients with complex needs and multiple long-term conditions Experience leading, supporting and developing clinical teams Experience of multidisciplinary working Experience of clinical governance, safeguarding and quality improvement initiatives Experience of discharge planning and community-based care pathways Strong clinical assessment and decision-making skills Excellent communication and relationship-building skills Ability to coordinate complex patient journeys and competing priorities Good report writing and digital literacy skills Full UK driving licence and access to a vehicle for work purposes, Previous experience within an Intermediate Care or Reablement service Experience leading operational service improvements Experience working across health and social care partnerships Additional leadership, management or quality improvement qualifications Knowledge of local health and social care systems and pathways We change lives by transforming health and care.
