Physicians provide a comprehensive range of primary and specialty care services at the 327-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus as well as at 10 Eskenazi Health Center sites located throughout Indianapolis.

FLSA Status Non-Exempt Job Role Summary The Clinical Oncology/Hematology Coordinator coordinates a collaborative, patient-centered model of care that provides a comprehensive clinical experience, addressing the physical, psychosocial and spiritual well-being of patients. This position facilitates the outpatient oncology/hematology care process and the follow-up care of those individuals who have been diagnosed with cancer or blood disorders.

The role is one of collaboration and coordination with referring community and internal physicians, nursing leadership, infusion staff, other clinical oncology nurses/coordinators, inpatient nurses and physicians, and other members of the health care team. Essential Functions and Responsibilities Evaluates the effectiveness of interventions and in collaboration with the multidisciplinary health care team makes appropriate revisions to the plan of care Completes all scheduling of new patients to oncology/hematology utilizing the scheduling software Functions as the main point of contact for referring community and internal physicians including communication regarding patient issues/treatment status, documenting any such contacts in the patient medical record Functions as the main point of contact for patients and families, assisting them with scheduling appointments, prescription refills, test result communication, or other health care needs via in-person or telephonic contact, documenting any such contacts in the patient medical record Obtains any pre-certification/pre-authorization needed for required services and medication Provides education throughout the continuum of care to patients, families, staff members and physicians as needed, including care for cancer-related pain, chemotherapy education, palliative care and hospice services Assists in the development, planning and implementation of oncology/hematology patient care protocols and clinic policies and procedures; assists in training staff as needed in policies and procedures Serves as the gatekeeper of clinical information and ensures compliance with federally mandated patient reporting; participates in performance improvement efforts and outcomes monitoring as needed Assists in operational troubleshooting and in streamlining work processes to assure quality outcomes and cost-effective care Responsible for the reporting and management of patient care data Participates on the Tumor Board Job Requirements Associate's degree in Nursing from an accredited college/university required; Bachelor's degree preferred One year of clinical nursing experience required; three years preferred Current Indiana license as an RN or eligibility for license in Indiana Knowledge, Skills & Abilities Advanced nursing skills: skills in operating specialized equipment, competence/accountability in preparing and administering medications and treatments according to policy and procedures, effective communication skills incorporating listening Effective interviewing techniques with patients and staff Knowledge of nursing theories, current clinical concepts, diseases/symptoms, trauma, treatments/medications, triage, appropriate diagnostic/therapeutic procedures and interventions Ability to function as member/leader of a health care team, delegate responsibilities/duties appropriately utilizing clinical expertise Ability to effect
