Grove Village Medical Centre is looking for an experienced and enthusiastic Clinical Pharmacist to join its friendly primary care team on a full time basis. The role offers a hybrid working arrangement with regular on site presence and will focus mainly on routine medicines management, medication reviews, long term condition medicines optimisation, prescription signing, audits and quality improvement work., The role will include: Conducting repeat medication and structured medication reviews Managing acute medication requests and prescribing queries Undertaking planned care reviews, including cardiovascular, renal and metabolic and diabetes reviews Reviewing hospital discharge letters and implementing medication changes Monitoring high risk medicines, including DMARDs Supporting safe and effective prescribing and medicines optimisation Providing clinical advice and support to patients and colleagues Working closely with our two existing Clinical Pharmacists, GPs, nurses and the wider practice team Contributing to the ongoing development and objectives of the practice, The practice is committed to supporting its clinical team and developing a collaborative working environment.
The Clinical Pharmacist will work closely with GPs, nurses, pharmacists and other healthcare professionals, with a particular focus on medicines management, medicines optimisation and supporting the delivery of high quality patient care. You will work closely with GPs, nurses, pharmacists and the wider practice team, helping to manage the routine pharmacy workload and support the delivery of safe, effective and cost efficient prescribing.
Previous primary care experience is desirable, with Independent Prescribing and SystmOne experience advantageous. Grove Village Medical Centre is a friendly and patient focused GP practice based in Bedfont, serving the local community and working as part of the wider primary care network. The practice provides a range of general practice and primary care services, with a multidisciplinary team working together to deliver safe, effective and high quality care to patients.
