Permanent job opening Seeking a board certified Internal Medicine or Family Medicine physician Lead Physician – Value-Based Primary Care Up to $300,000 base salary plus bonus Up to $50,000 sign-on bonus Provide primary care to a panel of medically complex and socially vulnerable adult patients Approximately 70-80% of time dedicated to direct clinical care Serve as a clinical leader alongside the Medical Director Lead an interdisciplinary care team focused on providing comprehensive, whole-person care Work collaboratively with physicians, APPs, nursing, medical assistants, behavioral health clinicians, social workers, care management, pharmacy, nutrition, and community health workers Coach and mentor physicians and advanced practice providers Provide clinical guidance to nurses, medical assistants, APPs, and other members of the care team Participate in population health huddles and help care teams achieve quality and utilization goals Manage and improve the health of patients with multiple chronic medical conditions Treat acute exacerbations of chronic conditions onsite when appropriate Accept same-day, next-day, and urgent appointments Provide transitional care for patients recently discharged from hospitals, emergency departments, and skilled nursing facilities Develop care plans designed to reduce unnecessary ER visits, hospital admissions, and readmissions Follow patients throughout the continuum of care, including hospital, ER, skilled nursing, home care, and outpatient settings Practice evidence-based medicine utilizing a patient-centered medical home (PCMH) model Participate in multidisciplinary meetings and recommend treatment initiatives designed to improve patient outcomes Track and achieve specified quality metrics Utilize technology and the EMR to improve quality initiatives and clinical efficiency Help develop clinical protocols and pathways that allow clinical support staff to practice at the top of their license Work with the Medical Director to develop partnerships with academic programs and community-based organizations Participate as a member of the center and market leadership team Help drive quality, utilization, patient engagement, and population health initiatives Represent the organization with community organizations, hospital systems, and payer partners On-call coverage shared on a rotating basis with other providers Active, unrestricted Ohio medical license required BLS certification required Clinical and administrative experience leading interdisciplinary teams required Training and mentoring experience required Experience in population health management preferred Experience working with medically and psychosocially complex patients strongly preferred Experience with the NCQA PCMH model preferred Must have excellent interpersonal, communication, organizational, and problem-solving skills Must be comfortable working in a collaborative, multidisciplinary environment Allow RecruitWell to set up a phone call with you and the client to discuss more!
