About the role You will be responding to those in need of social care support and undertaking the required work in accordance with the Care Act 2014 and Mental Capacity Act 2005. About the Teams Integrated Discharge Team - Watford General Hospital 37 hours per week | Permanent The Integrated Discharge Team at Watford General Hospital support a 7 day discharge service. This is a 37 hour a week position, and 1 in every 8 weekends you would be expected to work a Saturday and Sunday.
Our work is fast paced as we focus on supporting people to leave hospital as quickly and safely as possible, with a person-centred approach, with the majority of the work being through Discharge to Assess. The usual hours are 9am - 5pm and you would need to be able to work in Watford General Hospital every. Due to the nature of the hospital work, there is no remote or hybrid working in this role. The team is community based and you must have a full UK driving licence and regular access to a vehicle for working purposes Experience of working in social care We welcome applications from candidates who currently reside in the UK with established proof of right to work documentation.
We are not able to offer sponsorship at this time for this role., You'll need to have the ability to drive and/or travel around Hertfordshire, either using your own mode of transport or a company car to travel to other locations. English Fluency The ability to converse at ease with members of the public and provide advice in accurate spoken English is essential for the post (for those whose language is a signed language the provision of a sign language interpreter who speaks English to the necessary standard of fluency will be required).
