The postholder will hold responsibility for a defined caseload of patients drawn from a GP practice population. He/she will hold a specialist practice qualification and be skilled in assessment of patient care needs and in the development, implementation and evaluation of care packages and acquire and employ active nurse prescribing skills. He/she will work without supervision, and will, additionally, assist the Operational Lead by conducting delegated tasks instrumental in team management and care co-ordination.
The postholder will assume responsibility for directing team co-ordination. The postholder will lead in organising the induction and clinical supervision of students and new team members.
