We are looking for an enthusiastic, skilled and motivated Speech & Language Therapistto join our newly establishedEarly Supported Stroke Discharge (ESSD)team, delivering care acrossHigh Peak and North Dales,and also in the adult community SLT team across Chesterfield and North East Derbyshire. This is a rare opportunity to be part of our well established community SLT team as well as being integral to the development of a new service, working in line with the established and highly regarded ESSD model used across Derbyshire.
You will play a key role in enabling people to leave hospital earlier and recover safely and effectively in their own homes, delivering high-quality, evidence-based stroke rehabilitation. What you'll be part of Asupportive, friendly and progressive ESSD service and Adult Community SLT team The widerStroke and Neurology Service and Speech & Language Therapy Service within DCHS A multidisciplinary team with a strong commitment topatient-centred, evidence-based care A service that places training, development and staff wellbeing at its hear We are looking for a HCPC and RCSLT registered Speech and Language Therapist, with developed skills, working with communication impairments and dysphagia after stroke, as well as experience working with a community caseload including people with dementia and acquired neurological conditions.
You will be able to demonstrate leadership skills in developing service provision and clinical pathways relating to these specialisms. You will have a commitment to providing a high standard of patient focussed care and be a strong team player, with the ability to lead and influence others. You will be employed by Derbyshire Community Health Services NHS FT as part of their SLT service and as such have access to a team of colleagues with a range of skills and commitment to providing high standards of evidence based approaches, you will be supported by a SLT team leader.
We are a large and dynamic Speech and Language Therapy service of over 100 staff, working across all service areas in Derbyshire. Due to the nature of the role, applicants must be able to travel independently across the locality and have access to a suitable vehicle for business purposes. Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.
We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.
