Community Health Clinic (CHC) is seeking a compassionate, mission-driven Family Medicine Physician (MD/DO) to join our growing primary care team in Vandergrift, Pennsylvania. As a Federally Qualified Health Center (FQHC), CHC is committed to providing high-quality, comprehensive healthcare to underserved and underinsured individuals and families throughout our community. The Family Medicine Physician is responsible for providing comprehensive primary care services to patients of all ages while promoting continuity of care, preventive medicine, chronic disease management, and patient education.
The physician works collaboratively with an interdisciplinary healthcare team to improve patient outcomes and advance CHC's mission of providing accessible, patient-centered care. Essential Duties and Responsibilities Patient Care Provide comprehensive primary care services for patients of all ages. Obtain complete medical histories, including chief complaint, history of present illness, past medical history, family history, social history, and review of systems.
Perform comprehensive physical examinations utilizing appropriate clinical assessment techniques. Diagnose, treat, and manage acute and chronic medical conditions. Develop individualized treatment plans that include patient education, counseling, medications, diagnostic testing, preventive care, and follow-up. Order, interpret, and review laboratory tests, diagnostic studies, and imaging. Maintain accurate and timely documentation within the Electronic Health Record (EHR).
Coordinate referrals to specialists and community resources when appropriate. Provide follow-up care to evaluate treatment effectiveness and monitor disease progression. Participate in after-hours call coverage and emergency patient care as assigned. Clinical Leadership Participate in the coordination and continuity of patient care across the organization. Collaborate with Advanced Practice Providers by providing consultation, supervision, and medical record review as required.
Authorize patient care protocols utilized by Advanced Practice Providers. Assist with the development, implementation, and review of clinical policies and procedures to ensure compliance with regulatory and accreditation standards. Provide oversight of the CLIA-waived clinical laboratory in accordance with applicable regulations. Participate in Quality Improvement (QI), Quality Assurance (QA), and clinical performance initiatives.
Assist in monitoring clinical quality indicators and organizational healthcare goals. Participate in peer review activities and medical staff meetings. Community & Education Promote health education for patients and their families through individual and group educational opportunities. Collaborate with community agencies and healthcare partners to improve access to care and patient outcomes. Participate in continuing medical education and professional development activities.
Support the education and mentoring of clinical staff and healthcare providers. Represent Community Health Clinic in community outreach initiatives and organizational activities. Qualifications Required: Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) Current Pennsylvania Medical License (or ability to obtain prior to employment) Board Certified or Board Eligible in Family Medicine Current DEA Registration (or ability to obtain) Current BLS Certification Excellent communication and interpersonal skills Commitment to providing compassionate, pat
