Provide comprehensive primary care services under physician supervision Perform patient assessments, diagnoses, and treatment planning Prescribe and manage medications Deliver preventive, behavioral, and community health care Educate patients on disease management, medications, and preventive care Collaborate with an integrated, multidisciplinary care team Participate in after-hours on-call rotation Maintain accurate and compliant clinical documentation Participate in clinical quality improvement initiatives, peer reviews, and team meetings Provide cross-coverage for other clinicians as needed Job Responsibilities: Conduct patient assessments, diagnoses, treatment planning, and follow-up care.

Manage chronic conditions and acute medical concerns in a geriatric population. Utilize telehealth platforms to provide virtual consultations and follow-up visits. Collaborate with physicians, nursing staff, and care teams to ensure coordinated patient care. Maintain accurate and timely documentation in the electronic medical record system. Educate patients and caregivers on treatment plans, medications, and preventive care.

Qualifications: • Master’s degree or higher in Nursing (NP) or Physician Assistant studies • Licensed Nurse Practitioner (CA BRN) or Physician Assistant (CA PA Board) • Active DEA registration • Minimum 2 years of clinical experience • Background in Family Medicine, Internal Medicine, or Pediatrics preferred • Strong communication, clinical judgment, and teamwork skills • Bilingual skills preferred (Spanish, Mandarin, Cantonese, Tagalog, Farsi, or Vietnamese) Working Hours: • Full-time permanent position • Monday – Friday outpatient clinic schedule Skills: Strong clinical decision-making abilities Excellent communication and patient-care skills Compassionate bedside manner Ability to work independently and as part of a collaborative team A positive attitude, empathy, and a good sense of humor
