We are now seeking a passionate and motivated Higher Family Support Worker to join our Children with Disabilities Service Review Team; Supporting families to access the right services and interventions, ensuring care and support packages continue to meet the changing needs of children with disabilities.

This is a fantastic opportunity to play a key role in supporting children and young people with disabilities and their families, helping to ensure their needs are reviewed and met through effective partnership working. About the Role: The role of Family Support Worker within the Children with Disabilities Review Team is to work directly with children and young people with disabilities and their families, ensuring their needs are regularly reviewed and that support remains appropriate, effective, and responsive to changing circumstances.

Within the role, there will be opportunities to undertake training and develop knowledge of services, legislation, and best practice relating to children with disabilities. You will build strong relationships with families and partner agencies to promote positive outcomes for children and young people. The role is case holding, and as a key worker you will work alongside social workers, education professionals, health colleagues, and other agencies to review short breaks and support plans, identify unmet needs, and coordinate services.

You will be required to maintain accurate records, and provide updates that evidence progress, outcomes, and any areas requiring further support. You will join our Children with Disabilities Review Team and receive regular supervision and support from experienced managers to help you develop your skills and deliver high-quality services to children and families across Birmingham. The role requires travelling across the city to visit children, young people, and their families; therefore, access to a car is essential.

This is a full-time role working Monday to Friday during core hours, with flexibility to meet the needs of children and families. As a Family Support Worker, you will: Support children and young people with disabilities and their families to achieve positive outcomes by ensuring their assessed needs are regularly reviewed, and appropriate support is in place to promote their welfare, development, and wellbeing.

Support children and young people with complex and enduring disabilities, complex health and care needs, behaviours that may challenge, and those requiring palliative care, ensuring their needs are reviewed sensitively and that support remains safe, appropriate, and responsive to the child and family’s circumstances. Undertake the required training to support your role, including training related to children with disabilities, assessments, safeguarding, legislation, and multi-agency working.

Complete home visits, school visits to review short breaks, and review support plans for children and young people with disabilities and their families, as required, to inform short break plans and ensure services remain safe and responsive to changing needs and circumstances. Work collaboratively with families, social care colleagues, education, health professionals, and partner agencies to provide timely, child/young person-centred support and coordinate services that improve outcomes for children and young people with disabilities.

Develop and maintain specialist knowledge in areas relevant to children with disabilities, such as autism, learning disabilities, short breaks, transitions to adulthood, safeguarding, or direct payments, and share knowledge and best practice with colleagues. Maint
