Support the identification, assessment and monitoring of pupils with SEND and additional learning needs. Deliver, coordinate and evaluate targeted intervention programmes. Work collaboratively with teachers to implement effective strategies and provision that meet individual pupil needs across the curriculum. Contribute to the development, implementation and review of One Plans, SEND Support Plans and EHCP provision.
Liaise effectively with parents/carers, staff and external professionals to promote positive outcomes for pupils. Supervise and manage the SEND Hub, maintaining a purposeful learning environment and accurate, confidential SEND records.
