Responsibilities include: Providing direct patient care according to the established plan of care Completing skilled nursing visits in patients' homes Administering treatments and medications as ordered Monitoring patients and reporting changes in condition to the RN Educating patients and families on medications, treatments, and self-care Completing accurate and timely clinical documentation Following infection-control and safety procedures Collaborating with the RN, physicians, therapists, and other members of the care team Supporting patients in managing chronic conditions and maintaining independence at home Qualifications Current LPN/LVN license in Tennessee Graduate of an accredited practical or vocational nursing program Current CPR certification Reliable transportation Valid driver's license and current automobile insurance 3+ years of nursing experience preferred Home Health, Medical-Surgical, Community Health, or similar nursing experience preferred Why Home Health?
