Home Health LVN Assignment Duration This is a 13-week contract position. Schedule Monday to Friday, day shifts with 5 × 8 hour shifts per week. Location Atlanta, GA Job Description We are seeking a dedicated and experienced Licensed Vocational Nurse (LVN) for home health care. The ideal candidate will provide routine patient visits and ensure patients receive the highest quality of care. Responsibilities Conduct routine home health visits as required.
Complete necessary documentation and maintain patient records. Collaborate with healthcare team members to provide comprehensive patient care. Adhere to care plan and update it as necessary based on patient progress. Ensure compliance with health and safety regulations. Required Experience / Certifications / Licensure Current LVN license. Minimum of 32 patient points required per week. Recent home health experience is required.
