Provides outreach and enrollment services to meet the eligibility requirements of the program and the surrounding counties. Ensures coordination and planning with the help of community and family support. Focuses on the development and coordination of community service plans. Partners with service providers, families, and patients to create a plan of healthcare and identify additional service providers.

Responsible for overseeing customer service, supervising staff, and serving as a role model for staff, the daily assignment of staff, patient flow, and shift-to-shift decision making regarding staffing. Participates with leadership in the development of the case management standards, evaluation of outcomes, and development of improvement initiatives.Primary Responsibilities:-Provide day-to-day supervision to Case Managers and interns/students as appropriate.

-Complete psychosocial assessments covering psychological, social and environmental concerns, including evaluation of mental status. -Review daily referrals and screenings of clients. -Audit case file on a regular basis for completeness and accuracy.-Assist in discharge planning as required. -Serves and protects the hospital community by adhering to professional standards, hospital care policies and procedures, federal, state, and local requirements, and Joint Commission on Accreditation of Healthcare Organizations (JCAHO) standards.Qualifications Qualifications:Bachelor's Degree Nursing required or Master's Degree Social Work requiredLicensed Independent Clinical Social Worker (LICSW) State License in MA and NH required or Registered Nurse (RN) State License in MA and NH required3+ years supervisory or management experience with professional staff in a healthcare setting required3+ years of experience in case management, utilization review, or discharge planning requiredAdditional Knowledge, Skills and Abilities:- Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times.- Strong analytical ability, critical thinking and processing skills.- Strong customer and interpersonal skills.- Strong teamwork skills.- Excellent organizational and communication skills.- Ability to deal with rapid changing health care environment and Requires excellent interdisciplinary team collaboration skills.Additional Job Details (if applicable) Schedule and Work Model:Full-time, Monday through Friday, standard business hours.Hybrid work Model: Approximately 2-3 days per week on-site at Wentworth Douglass Hospital in Dover, NH and approximately 2-3 days remote from home.On remote workdays, employees must use a stable, secure, and compliant workstation in a quiet environment.
