With a varied workload and no two days the same, it is ideal for someone who enjoys building relationships and contributing to innovative healthcare services.
As a Care Coordinator, you will work with PCN leaders, practice teams and partner organisations to improve patient care and access to services. You will support patients with personalised care needs, helping them navigate health and community services and access the right support at the right time. You will help promote the work of the PCN through social media, patient engagement activities and health campaigns, raising awareness of local services, screening programmes, immunisations and wellbeing initiatives.
You will also support the Patient Participation Group, gather patient feedback and contribute to service improvements. Working closely with the PCN Manager, Clinical Director and partner organisations, you will support quality improvement projects, audits and healthcare programmes while developing partnerships that enhance integrated patient care. Undertake work in line with PCN directed priorities. 2. Manager PCN Social Media 3. Proactively identify and work with a cohort of people to support their personalised care requirements, using the available decision support aids 4.
