In accordance with NL Health Services's vision and values and the Department of Health and Community Services Health Home Model of Team-Based Care, the Regional Primary Health Care Manager - Collaborative Team Clinic, is responsible for effective leadership, resource management, program operational planning, quality, access systems, and service delivery; client/community relationships; and quality improvement, evaluation, and risk management of Primary Care services to support patients and providers in the Collaborative Team Clinic.
The Regional Manager works as a team member with internal and external stakeholders and supportive programs in addition to cross-sector health teams, providers, and other partners/agencies in a client and family-centered approach. The Manager coordinates a systematic, standardized Primary Health Care approach in NL Health Services Primary Health Care collaborative team clinic sites and primary health care networks using the five principles of Primary Health Care which include access, continuity, person-focused, collaborative and team-based and engaged communities thereby improving quality of care in an effective, efficient and equitable manner.
The manager provides direct supervisory, administrative and operational direction to front line staff of the Primary Health Care and Chronic Disease Prevention and Management program. The manager will also provide leadership and facilitation of interdisciplinary team development, inter-sectoral collaboration, change management processes and community-capacity building. The incumbent will maintain strong working relationships with NL Health Services departments and programs, other service providers and the community.
Job Qualifications Education Bachelor's degree in a health-related discipline is required. Master's degree is preferred. Experience Minimum of five (5) years' experience within the last seven (7) working in Acute, Long-Term Care or Community Care is required. Considerable experience in a leadership role within Acute, Long-Term Care or Community Care is required. Experience in leading collaborative teams is preferred.
Experience in stakeholder and community engagement is preferred. Competencies (Knowledge, Skills & Abilities) Demonstrated knowledge and proficiency in the Microsoft Office Suite of applications. Demonstrated excellent interpersonal, organizational and leadership skills. Demonstrated exceptional oral and written communication skills Demonstrated strong analytical, facilitation and negotiation skills together with sound judgment, diplomacy Demonstrated ability to establish and maintain respectful working relationships with clients/patients/residents, staff, families, and the community.
Demonstrated knowledge and experience in organizational management, client centered care and working with communities Demonstrated knowledge of the Health Home Model of Team-Based Care and the principles of Primary Health Care.
