• Full-time opportunity for an experienced Nephrology Nurse Practitioner (NP) or Physician Assistant (PA) • Provide a combination of outpatient nephrology care and inpatient hospital coverage • Manage patients with chronic kidney disease, hypertension, electrolyte disorders, and other renal conditions • Provide hospital consultations, rounding, and dialysis management • Work collaboratively with nephrologists, physicians, nurses, dialysis teams, and other healthcare professionals • Opportunity to work within a well-established, physician-owned multi-specialty medical group • Flexible scheduling and strong work-life balance Job Responsibilities: • Evaluate and manage patients with kidney disease and related conditions • Provide outpatient nephrology services in New Windsor and Monroe, NY • Perform inpatient nephrology consultations and hospital rounds • Conduct daily dialysis rounds and manage acute and chronic dialysis patients • Order and interpret laboratory tests, imaging, and other diagnostic studies • Develop and implement individualized treatment plans • Collaborate with nephrologists and multidisciplinary healthcare teams • Educate patients and families regarding kidney disease, treatment, diet, and dialysis • Maintain accurate and timely medical records • Participate in quality improvement initiatives Qualifications: • Current New York State NP or PA license • National certification appropriate to the provider's discipline • Master's or doctoral degree as applicable • Valid DEA license • Minimum 2 years of Nephrology experience required • Experience with both outpatient and inpatient nephrology care • Dialysis management experience required • Strong clinical assessment and diagnostic skills • BLS/ACLS certification preferred Working Hours: • Full-time position • Flexible scheduling available based on practice needs • Outpatient clinic coverage in New Windsor and Monroe • Inpatient hospital rounding and dialysis coverage • Schedule can offer flexibility with preferred days and start/end times Skills: • Strong nephrology and dialysis knowledge • Excellent patient assessment and clinical decision-making skills • Ability to manage acute and chronic kidney conditions • Strong communication and interpersonal skills • Ability to work independently and collaboratively • Excellent patient education skills • Strong organizational and documentation skills • Familiarity with EMR systems preferred
