You will work in a collaborative, patient-centered environment that emphasizes longer visits, same-day/next-day access, cost-effective care, and continuity between patients and their care team. The ideal clinician brings 2–4 years of clinical experience and a passion for providing high-quality, relationship-based care. This opportunity offers the ability to practice medicine in a model designed to reduce administrative burden while improving patient outcomes and provider satisfaction.
Position Responsibilities Provide comprehensive primary care services for patients across the lifespan Perform physical examinations and comprehensive health assessments Diagnose and treat acute illnesses, chronic conditions, and preventive care needs Order, interpret, and review laboratory tests, imaging studies, and diagnostic evaluations Develop individualized treatment plans based on evidence-based clinical practices Prescribe medications and therapies within scope of practice Manage chronic disease conditions including hypertension, diabetes, obesity, COPD, and other common primary care diagnoses Educate patients and families regarding treatment plans, preventive care, wellness, and lifestyle modifications Coordinate referrals and specialty consultations as appropriate Maintain accurate and timely documentation within the electronic medical record Collaborate closely with physicians and interdisciplinary care team members Support population health initiatives and preventive medicine programs Deliver care through in-person visits and telehealth modalities when appropriate Qualifications Required: Graduate of an accredited Nurse Practitioner or Physician Associate program Active Wisconsin NP or PA license (or ability to obtain) National certification appropriate to specialty Strong clinical judgment and decision-making skills Excellent communication and patient relationship-building abilities Preferred: 2–4 years of clinical experience Primary Care experience Family Medicine experience Urgent Care experience Occupational Health experience Direct Primary Care experience Ideal Candidate The successful candidate will: Be passionate about patient-centered primary care Enjoy developing long-term relationships with patients Value prevention, wellness, and chronic disease management Thrive in a collaborative, physician-led environment Demonstrate strong communication and interpersonal skills Be comfortable practicing with a high degree of autonomy Focus on delivering cost-effective, evidence-based care Support innovative healthcare models that improve access and outcomes Practice Highlights Physician-led organization Team-based care model Direct Primary Care environment Longer patient visits Same-day and next-day patient access Focus on preventive care and chronic disease management Reduced administrative burden Strong provider-patient continuity Collaborative clinical support structure In-person and telehealth care delivery
