Job Posting: Nurse Practitioner – Region 5 Location: Mount Vernon, OH 43050 Duration: At least 1 year from projected start date Hours/Week: Up to 24 hours Job Summary We are seeking an experienced Nurse Practitioner to provide advanced nursing care, including assessment, diagnosis, treatment, and ongoing management of patients with acute and chronic conditions. This role operates in accordance with Ohio state licensure and established clinical guidelines.

Key Responsibilities 1. Patient Care & Clinical Duties • Create and implement individualized patient care plans within the scope of licensure. • Monitor patient progress and intervene in the management of chronic illnesses. • Perform routine physical examinations to assess chronic conditions and diagnose new conditions. • Evaluate acute and chronically ill patients and prescribe medications or administer treatments.

• Complete discharge planning and follow-up care for patients with medical or chronic conditions. • Perform minor surgical procedures as permitted by training and competency. • Determine the need for specialty referrals. • Order, review, and interpret laboratory tests, X-rays, and other diagnostic studies. • Administer immunizations and vaccinations. 2.

Medical Documentation • Accurately document patient encounters in compliance with policies, protocols, and professional standards. • Record assessments, diagnostic impressions, treatment plans, and follow-up care. • Participate in ongoing medical quality oversight activities. 3. Clinical Leadership • Provide clinical guidance and direction to nursing and allied healthcare staff.

4. Professional Participation & Development • Participate in clinical meetings and quality improvement initiatives. • Serve as a clinical resource for patients and families regarding health maintenance and risk factors. • Attend required trainings, in-services, and continuing education programs. ________________________________________ Required Knowledge, Skills & Abilities • Strong knowledge of advanced nursing practices and HIPAA regulations.

• Familiarity with state and federal healthcare regulations. • Proficiency in using standard medical equipment and computer systems. • Excellent clinical judgment, analytical, and problem-solving skills. • Ability to maintain confidentiality and build positive relationships with patients and healthcare staff. ________________________________________ Minimum Qualifications • Active Ohio Registered Nurse (RN) license.

• Completion of an accredited Nurse Practitioner program. • Certified Advanced Practice Registered Nurse (APRN) in Ohio. • Valid Certificate of Authority and Certificate to Prescribe per ORC 4723. ________________________________________
