An opportunity has arisen for a Nurse Practitioner to join our friendly and supportive team. We are looking for a highly motivated and enthusiastic individual, ideally with experience of working within primary care, to work alongside and support our GPs and existing Nurse Practitioners. The successful candidate will have the opportunity to work across a varied and rewarding role, seeing patients with minor illness and acute presentations, supporting chronic disease management, and undertaking home visits as required., The successful candidate will play an integral role within our primary care team, providing high-quality, patient-centred care across a varied caseload.
The role will include the assessment, diagnosis and management of patients presenting with minor illness and acute conditions, alongside supporting the management of patients with long-term conditions. The successful candidate will also undertake home visits where required., We are seeking a Nurse Practitioner who will provide high-quality, patient-centred care to patients presenting with a wide range of acute and ongoing healthcare needs.
The role will include: Assessing, diagnosing and managing patients presenting with minor illness and acute conditions, working autonomously within their scope of practice. Providing appropriate clinical advice, treatment and safety-netting to patients, ensuring they receive the right care at the right time. Supporting patients with their ongoing medical and nursing needs, including contributing to the management and review of patients with long-term conditions.
Requesting and interpreting appropriate investigations and making referrals to other healthcare professionals and services where clinically indicated. Undertaking home visits as required, providing assessment, diagnosis, treatment and ongoing management to patients who are unable to attend the practice. Working collaboratively with GPs, Nurse Practitioners, Practice Nurses, Healthcare Assistants and the wider multidisciplinary team to ensure continuity and high standards of patient care.
Promoting health, wellbeing and self-management, whilst ensuring patients are fully involved in decisions about their care. Maintaining accurate and comprehensive clinical records and working in accordance with practice policies, professional standards and relevant clinical guidelines. Candidates should hold an Independent Prescribing qualification and be confident in assessing, diagnosing and managing patients autonomously within their scope of practice.

Confidentiality is integral to this role, and the successful candidate will be expected to maintain the highest standards of confidentiality, professionalism and patient dignity at all times., BSc (Hons) Nurse Practitioner Degree or equivalent qualification, or working towards Clinical Skills Level 7 Paediatric Clinical Skills or undertaking studies towards Independent/Supplementary Nurse Prescriber, Excellent interpersonal skills Ability to network Excellent verbal and written communications skills Ability to develop strong and effective working relationships Able to demonstrate and high level of personal, professional accountability and autonomy Ability to manage own time effectively Coxhoe Medical Practice is part of a well-established and forward-thinking practice.
