Job Responsibilities ✅ Provide comprehensive outpatient primary care to patients across the community ✅ Manage chronic conditions such as diabetes, hypertension, asthma, COPD, obesity, and other common primary care needs ✅ Conduct wellness visits, preventive screenings, follow-ups, medication management, and patient education ✅ Collaborate with APPs, nurses, behavioral health, care coordinators, and other clinical support staff ✅ Support quality initiatives focused on access, prevention, patient engagement, and improved health outcomes ✅ Maintain accurate and timely documentation in the EMR ✅ Provide culturally aware, patient-centered care to a diverse population Qualifications ✅ Board-certified or board-eligible Family Medicine or Internal Medicine Physician ✅ Active California medical license, or ability to obtain ✅ Outpatient primary care, family medicine, internal medicine, community health, or FQHC experience preferred ✅ Comfortable caring for diverse patient populations ✅ Strong communication skills and a collaborative, team-based approach ✅ Commitment to quality care, access, prevention, and community health Working Hours ✅ Full-time outpatient schedule ✅ Monday–Friday schedule ✅ No hospital rounding ✅ Supportive clinical team environment ✅ Paid holidays and vacation ✅ Stable outpatient practice model Skills ✅ Family Medicine or Internal Medicine ✅ Outpatient primary care ✅ Chronic disease management ✅ Preventive medicine ✅ Community health ✅ Patient education ✅ EMR documentation ✅ Collaborative care-team communication ✅ Spanish-speaking ability preferred Compensation & Benefits This opportunity offers a highly competitive compensation package with earning potential up to $350,000 , including base compensation and bonus potential.
