We are seeking an enthusiastic and experienced Paramedic Practitioner to join our Acute Home Visiting Service, delivering high-quality care to patients in their own homes across the Primary Care Network (PCN). Working autonomously within a multidisciplinary team, you will assess, diagnose, treat and manage patients with acute illness, frailty and complex health needs who are unable to attend surgery.

Using advanced clinical skills, you will develop management plans and facilitate admission, referral or community-based care where appropriate. This is a part-time role working 10:00am to 6:00pm. Working days will include every Friday, which is fixed due to service demand, together with either Wednesday or Thursday, to be agreed with the successful candidate. Working closely with GPs, community teams, care homes and secondary care services, you will deliver integrated care, support patients to remain safely at home and help prevent avoidable hospital admissions.

You will also contribute to service development, quality improvement and patient education., The Paramedic Practitioner will provide autonomous, patient-centred care within the Primary Care Network, delivering acute home visits and supporting patients within both community and practice settings. Working as part of the multidisciplinary team, the postholder will assess, diagnose, treat and manage patients presenting with acute and long-term health needs, developing appropriate management plans and facilitating referral where required.

The role includes undertaking urgent home visits, avoiding unplanned admission reviews, prescribing within competence, supporting vaccination programmes and chronic disease management for housebound patients. The postholder will promote self-care and patient education, maintain accurate clinical records, participate in audit and quality improvement activities, and work collaboratively with colleagues to ensure safe, effective, evidence-based care.

Central and West Warrington Primary Care Network (CWW PCN) is a collaboration of seven GP practices serving approximately 68,000 patients across Central and West Warrington. The PCN works closely with GP practices, community services, and multidisciplinary healthcare professionals to provide accessible, high-quality, and integrated care to the local population. Through a range of additional roles, including Paramedics, Clinical Pharmacists, Physiotherapists, Social Prescribers, Care Coordinators, and Health and Wellbeing Coaches, the PCN aims to improve patient access to services, support the management of long-term conditions, reduce health inequalities, and help patients remain healthy and independent within their communities.
