Seeking an experienced Nurse Practitioner to provide onsite primary care services for adult and geriatric patients in community-based settings. Enjoy a flexible weekday schedule with no nights, weekends, or on-call responsibilities. Job Responsibilities: • Perform comprehensive patient assessments, examinations, and evaluations • Diagnose and manage acute and chronic medical conditions • Order, interpret, and document diagnostic tests and studies • Prescribe medications and treatment plans in accordance with regulations • Provide patient education on disease prevention, treatment plans, and wellness strategies • Collaborate with physicians, nurses, and multidisciplinary healthcare teams • Perform advanced clinical procedures and emergency interventions when necessary • Maintain accurate and timely electronic medical records • Ensure compliance with all clinical, regulatory, and quality standards Qualifications: • Active New York Nurse Practitioner license • Current DEA registration • Active Medicare & Medicaid provider enrollment • Must meet New York's 3,600-hour independent practice requirement • Primary care experience • Master's Degree in Nursing (MSN) Working Hours: Part-Time (16–20 Hours/Week or 3 Weekdays) Skills: • Geriatrics, Internal Medicine, or Family Medicine experience • Assisted living, long-term care, or post-acute care experience • Strong communication, documentation, and clinical decision-making skills
