(Part Time)👨‍⚕️ Family Medicine Physician – Assisted Living Primary Care - Hoosick Falls, NY 12090

• Seeking an experienced Family Medicine or Internal Medicine Physician to provide primary care services to residents in assisted living facilities throughout the Albany region. • Opportunity available on a part-time or full-time basis with flexible scheduling. • Focus on delivering comprehensive, patient-centered care to a geriatric population in a collaborative environment.

• Build meaningful, long-term relationships with residents while improving quality of life and health outcomes. Job Responsibilities: • Provide comprehensive primary care evaluations and follow-up visits. • Diagnose, treat, and manage acute and chronic medical conditions. • Develop and implement individualized treatment plans. • Review laboratory and diagnostic results and adjust treatment plans as needed.

• Coordinate care with facility staff, specialists, and family members. • Maintain accurate and timely EMR documentation. • Prescribe medications and monitor treatment effectiveness. • Participate in preventive care and wellness initiatives. Qualifications: • MD or DO degree from an accredited medical school. • Active New York State Physician License. • Active DEA Registration. • Currently enrolled and credentialed with Medicare and Medicaid (required).

• Primary Care experience required. • Geriatric or Long-Term Care experience preferred. Working Hours: • Part-Time: 2–3 full days per week. • Flexible scheduling available. • Expected start within 4–6 weeks following credentialing and onboarding.
