Job Responsibilities: • Provide comprehensive primary care services under physician supervision • Perform patient assessments, diagnoses, and treatment planning • Prescribe and manage medications • Deliver preventive, behavioral, and community-based healthcare • Educate patients on disease management and preventive practices • Collaborate with multidisciplinary care teams • Maintain accurate and compliant EMR documentation • Participate in on-call rotation and provide cross-coverage as needed • Engage in quality improvement initiatives, peer reviews, and team meetings Qualifications: • Master’s degree or higher in Nursing (NP) • Active California Nurse Practitioner license (BRN) • Active DEA registration • Minimum 2 years of clinical experience • Background in Family Medicine, Internal Medicine, or Pediatrics preferred • Strong clinical judgment and communication skills • Ability to work effectively in a team-based environment • Preferred: Bilingual (Spanish, Mandarin, Cantonese, Tagalog, Farsi, or Vietnamese) Working Hours: • Full-Time Schedule • Outpatient clinic hours (typically Monday – Friday) • Rotational after-hours on-call responsibilities Skills: • Strong clinical assessment and diagnostic abilities • Effective patient communication and education skills • Proficiency in EMR/EHR systems • Team collaboration and coordination • Cultural competency and patient-centered care approach • Organizational and time management skills
