We are seeking a dedicated Nurse Practitioner to provide comprehensive geriatric-focused care within skilled nursing facilities in Butte, Montana. This is an excellent opportunity to work closely with a collaborative healthcare team while making a meaningful impact on the lives of elderly patients. • Full-time opportunity • Practice setting: Skilled Nursing Facilities (SNF) • Geriatric-focused patient population • Collaborative and supportive clinical environment • Opportunity to provide hands-on, patient-centered care Job Responsibilities: • Perform comprehensive patient assessments, histories, and physical examinations • Diagnose and manage acute and chronic medical conditions • Order, interpret, and document diagnostic tests and studies • Develop and implement evidence-based treatment plans • Prescribe medications and therapies as appropriate • Coordinate specialty referrals and follow-up care • Educate residents and families regarding diagnoses, treatment plans, and preventive care • Collaborate daily with multidisciplinary SNF teams, peer providers, and supervising physicians • Maintain accurate and timely medical documentation Qualifications: • Active Nurse Practitioner license • Board Certification required • Experience in geriatrics, primary care, internal medicine, or SNF settings preferred • Strong communication and patient management skills • Ability to work independently and collaboratively within a team environment Working Hours: • Full-Time position • Flexible scheduling options available
