Job Title Physical Therapist Location Newport News, VA | Home Health Schedule / Job Type Contract | Days | 5x8 Hours (8:00–16:30) Key Responsibilities Provide physical therapy services to home health patients in accordance with plan of care Complete and maintain OASIS assessments as required for home health documentation Chart and document patient progress accurately and in a timely manner Travel to patient locations within the assigned service area; mileage reimbursed at IRS rate Float to other locations as needed based on operational needs Required Qualifications Active Physical Therapist license; Virginia license required to be in hand at submittal or valid compact license with proof provided Home health experience required OASIS experience required Specialty experience within the last year required SSN required at time of submittal One supervisory reference required at time of submittal Local candidates are welcome and accepted at the same rate Preferred Experience / Skills Familiarity with travel assignment compliance expectations and scheduling flexibility Experience managing patient documentation efficiently in a field-based setting Why Join Us Mileage reimbursed at the IRS standard rate, supporting cost-effective travel to patient locations Opportunity to work a consistent day schedule in a rewarding home health environment
