We reduce administrative burdens and support our affiliated clinicians in focusing on what matters most—delivering high-quality care. With 140+ independently contracted providers and growing, Trumen serves over 145 facilities across Texas, including Austin, Houston, San Antonio, Killeen, and Bryan/College Station. Job Responsibilities: Job Responsibilities Core Responsibilities: • Provide in-person medical care to patients in designated post-acute facilities • Perform assessments, manage treatment plans, and maintain accurate documentation in the EMR • Collaborate with facility teams to support quality outcomes and smooth transitions of care • Collaborate as applicable in Trumen’s value-based care and transitional care programs • Develop strong working relationships with facility and clinical staff • Serve as a liaison between Trumen and facility teams • Maintain an on-site presence consistent with your agreed schedule and facility needs Qualifications: Qualifications Qualifications: • MD or DO with a current, unrestricted Texas medical license • Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine • 1+ year of experience in post-acute, geriatric, or SNF care preferred • Comfortable practicing independently in skilled nursing facilities • Strong clinical judgment, communication skills, and a patient-centered mindset
