We are seeking a Nurse Practitioner to provide geriatric-focused care in skilled nursing facilities across Seattle, Washington. This role offers a collaborative, team-based environment with a strong focus on quality care, allowing providers to build meaningful relationships with patients while maintaining a balanced workload. Job Responsibilities: • Provide comprehensive geriatric care in SNF settings • Perform patient evaluations, histories, and physical examinations • Order, interpret, and document diagnostic studies • Develop and implement evidence-based treatment plans • Prescribe medications and therapies • Manage acute and chronic conditions • Coordinate referrals and interdisciplinary care • Educate residents and families on care plans and health maintenance • Collaborate with physicians and care teams for optimal outcomes Qualifications: • Licensed Nurse Practitioner (NP) • Active Washington state license (or ability to obtain) • DEA license required • Experience in geriatrics or SNF preferred • Strong clinical and communication skills Working Hours: • Full-time schedule • SNF-based setting • Daytime, onsite care Skills: • Geriatric and chronic disease management • Patient-centered care approach • Strong clinical assessment skills • Team collaboration and coordination • EMR documentation proficiency
