We are currently seeking Family Medicine, Internal Medicine, or Geriatrics providers to join a PACE (Program of All-Inclusive Care for the Elderly) setting in Cambridge, MA . This opportunity is open to Physicians, Nurse Practitioners, and Physician Assistants who are passionate about caring for complex older adults and medically fragile patients . The PACE model focuses on coordinated, patient-centered care , allowing providers to work closely with interdisciplinary teams to improve the health and quality of life of elderly participants.

🌟 Position Highlights ✨ Work in a unique PACE care model focused on elderly and complex patients ✨ Opportunity for Physicians, Nurse Practitioners, and Physician Assistants ✨ Balanced weekday schedule with no call responsibilities ✨ Collaborative interdisciplinary care team environment ✨ Utilize Epic EMR for seamless documentation and patient management ⏰ Schedule 🗓 Assignment Dates: May 1, 2026 – November 30, 2026 📅 Schedule: Monday – Friday 🕗 Hours: 8:00 AM – 5:00 PM 📞 Call: No call required This schedule allows for a consistent work-life balance while delivering high-quality patient care.

👩‍⚕️ Clinical Responsibilities 🔹 Provide comprehensive primary care for PACE program participants 🔹 Manage chronic and acute medical conditions 🔹 Conduct clinic visits, home visits, and nursing home visits as assigned 🔹 Participate in interdisciplinary care team meetings 🔹 Coordinate care with nursing, social workers, therapists, and support staff 🔹 Manage patient communications such as phone and in-basket coverage 🔹 Document patient encounters using Epic EMR 👥 Patient Population PACE programs serve elderly patients and individuals with complex medical conditions who require coordinated long-term care.

📊 Typical Patient Panel: Approximately 70 patients 📌 Patient volumes are smaller due to higher complexity of care This allows providers to focus on quality care and comprehensive patient management. 🏥 Practice Environment 🤝 Work alongside physicians, nurse practitioners, and physician assistants 👩‍⚕️ Strong interdisciplinary care teams including nurses, therapists, and social workers 🏥 Combination of clinic, home, and nursing facility care settings 💻 Epic EMR system ✅ Requirements ✔ MD / DO – Family Medicine, Internal Medicine, or Geriatrics (for physicians) ✔ Nurse Practitioner or Physician Assistant certification (for APPs) ✔ Board Certified (for physicians) ✔ Active Massachusetts Medical License required ✔ Flu vaccination required (no waivers accepted) ✔ Recent direct supervisor reference required 🌆 Why Cambridge, MA?
