Physical Therapist I/II / Advanced Physical Therapist I/II Location Northwestern Arizona Assignment Duration This is a 13-week contract position. Schedule Monday – Friday, 8 AM – 5 PM, with weekend rotation. Job Description The Physical Therapist I/II / Advanced Physical Therapist I/II provides quality physical therapy services primarily to a Native American population for the treatment or prevention of physical disability or disease.

This role is responsible for managing and implementing the Physical Therapy program, which includes: Responsibilities Provision of physical therapy evaluation and treatment for inpatients and outpatients, ages ranging from birth through geriatric. Delivery of specialty services such as wound care, prosthetic and orthotic clinic, geriatric clinic, high school clinic, functional clinic, spinal cord clinic, and assistive technology/adaptive equipment evaluation/treatment.

Management of the Durable Medical Equipment Prosthetic Orthotic Supply Program. Assumption of direction and development of the physical therapy section to include continuing quality improvement, risk management, and safety. Preparation for Joint Commission accreditation and participation in staff education and reporting, including staff meetings and in-services. Required Experience / Certifications / Licensure Education: Bachelor’s degree from a Commission of Accreditation in Physical Therapy Education (CAPTE) accredited school of Physical Therapy.

License: Must possess a valid, current, full, and unrestricted license in any state within the United States, District of Columbia, or the Commonwealth of Puerto Rico. Certification: Must have and maintain a current Basic Life Support (BLS) certification by the American Heart Association. Experience: At least 3 years of experience as a licensed Physical Therapist, with outpatient experience required.
