Manage and coordinate a caseload of hospice patients and families Complete patient assessments, admissions, care planning, and ongoing evaluations Develop individualized care plans that address physical, emotional, social, and spiritual needs Collaborate closely with physicians, social workers, aides, chaplains, and other interdisciplinary team members Conduct routine patient visits and adjust care plans as needs change Provide education, guidance, and support to patients and their families Perform clinical pronouncements and offer compassionate support during end-of-life transitions Maintain accurate and timely clinical documentation Supervise and support Home Health Aides in accordance with state, federal, and company guidelines Qualifications Graduate of an accredited Registered Nursing program Active Registered Nurse (RN) license in the state of practice Current CPR and First Aid Certification Knowledge of quality assessment and quality assurance practices Strong clinical judgment, communication, and patient advocacy skills Valid driver's license and proof of insurance Find Purpose in Every Visit At Miracle City Hospice, nursing is more than a career.
