Acute Care Case Manager Assignment Duration This is a 13-week contract position. Schedule Day Shift: 5×8 Hours (08:00 – 16:30) Job Description We are seeking an experienced Acute Care Case Manager for an immediate position. The ideal candidate will be seasoned with a strong background in acute care, case management, and discharge planning within a hospital setting. This role requires engagement in care coordination and close collaboration with multidisciplinary teams to ensure optimal patient outcomes.
Responsibilities Assess patients’ needs and develop coordinated care plans. Manage discharge planning processes to ensure safe transitions. Participate in family meetings and facilitate communication between patients and their care team. Conduct utilization review and manage prior authorizations. Coordinate with various departments including Social Services and Physical Therapy. Required Experience / Certifications / Licensure Minimum of 2 years of Acute Care Case Management experience required.
California state license is necessary; pending licenses accepted if fully licensed by start date. Experience with healthcare organizations including HMOs or IPAs is a plus.
