Case Manager (Registered Nurse) General Information Minimum Experience: 1.5 years of general nursing experience. Travelers must reside more than 100 miles away from any facility within the region and must have over 2 years of experience if they are first-time travelers. Permanent staff are required to wait one full year before returning as travelers. Work Hours Standard hours are from 08:00 AM to 4:30 PM.
Patient Population Case Managers provide care across all acute care departments. The RN Case Manager will primarily remain in the same area throughout their contract, pending any new hires. Travelers are typically not required to float between multiple areas during their assignment. Patient Ratios 1 Case Manager per 16-20 patients. Required Certifications BLS Certification. Education Requirements Completion of a nursing education program that is board-approved.
Skills Required Previous experience in discharge planning and Acute Care Case Management. Proficient in the progression of care. Experience in collaborating with families to discuss care plans. Support on the Unit Case Management Assistants Lead Case Managers Social Work – Complex Psychosocial Patient Consultants Utilization Review RNs Charting System EPIC (required). Orientation Orientation comprises 1.5 weeks, including EPIC training.
Scheduling Weekend rotation includes working 2 weekends per 13-week assignment. Staffing support during weekend shifts typically includes: 3 RN Case Managers 1 Social Worker 1 Lead Case Manager 1 Case Management Assistant 1 Utilization Review Nurse Holiday Expectations Requires working 2 out of 4 major holidays. Time Off Approval for time off includes up to 7 days, with extensions required for any full week off.
No requests for time off will be honored during the Sturgis Bike Rally (first 2-3 weeks in August). Additional Information Scrub Color: Navy Blue or professional attire. Free parking available.
