Our short-term services are recruiting for a Support Worker to join our team on a part-time (36 hrs) permanent contract were you will provide care and support to people in the community with a focus on maximising independence, and delivering a high standard of care to a variety of people, this can also include end of life care. Following a period of acute ill health or hospitalisation, you will undertake short term care as defined in care & short term service plans or delegated task from Therapist/Nurses, and contribute to the assessment process and ongoing monitoring and review of peoples goals and reporting any changes to a persons medical condition.

This is an exciting development of services to help prevent avoidable hospital admissions working alongside intermediate care teams and other community teams., You will be working as part of a team, this will include lone working within the community. You will be working across Torbay and South Devon to provide integrated health and social care. Working closely with other organisations. Provide support to service users with personal care, domestic care, social needs and clinical needs in accordance with the care plan, and within the limits of own competencies and responsibilities To observe and evaluate the service users response to the care plan and communicate changes in need and/or circumstances to the appropriate member of the service in a timely manner Provide a flexible service which will include shift patterns between 0800 hrs and 2200 hrs, 7 days a week Provide reports on service users using the service IT system , while complying with the Trust Policies and Procedures Contribute to a safe working environment for self and others and report situations on accordingly in line with Trust Policies and Procedures and other partner organisations Undertake training relevant to the post and in line with personal development and review During times of reduced work to provide support to other teams linked with the service Please note: Ability to travel efficiently around the Bay is important in order to carry out your duties (e.g.

Ability to travel efficiently around the Bay is important in order to carry out your duties (e.g. Full driving licence and vehicle). Training and Benefits: You'll receive full training in short-term services along with a provided uniform. Each shift is paid in full, regardless of direct contact time, and travel time is paid at your full hourly rate with a generous mileage allowance. We also offer shift enhancements for evenings, weekends, and bank holidays.
