About the Role As a Nurse Practitioner or Physician Assistant, you will provide comprehensive, patient-centered care across post-acute and long-term care settings. You’ll collaborate with a multidisciplinary team to improve outcomes, support recovery, and enhance quality of life for your patients.

This is an ideal role for clinicians who enjoy autonomy, continuity of care, and working with a diverse patient population. Care Settings Post-Acute Care Long-Term Care Skilled Nursing Facilities Assisted Living Memory Care Key Responsibilities Provide comprehensive medical care and ongoing patient management Conduct assessments, diagnoses, and treatment planning Manage chronic conditions and acute medical changes Collaborate with physicians and interdisciplinary teams Participate in care planning and team discussions Maintain accurate and timely documentation Focus on patient-centered, quality-driven outcomes Work-Life Balance Monday–Friday schedule (8 AM – 5 PM) No evenings, weekends, holidays, or on-call duties Flexible workflow with strong clinical autonomy Compensation & Benefits Competitive salary Sign-on or relocation bonus (up to $5,000) Generous Paid Time Off 401(k) with 3% company match Health, Dental, and Vision Insurance $2,000 annual CME allowance License renewal reimbursement Malpractice insurance coverage Qualifications Licensed Nurse Practitioner or Physician Assistant Active Oregon license (or ability to obtain) Background in Family, Adult, or Geriatric care preferred Post-acute or facility-based experience is a plus (not required) New graduates welcome to apply
